- Your Candidate Document Number (CDN), which is required to process your form. For information on obtaining a CDN, click here.
- Valid photo identification, such as a driver's license or passport.
- Any previous Marine Medical Certificate you may have, including details of past restrictions.
- A list of all medications you are currently taking.
- Your glasses or contact lenses, if applicable.
